Transcatheter Interatrial Shunt Device for the Treatment of Heart Failure

چکیده
Prevalence of Heart Failure With Preserved Left Ventricular Ejection Fraction More than one-half of people with heart failure (HF) have a preserved left ventricular ejection fraction (HFpEF). Patients with HFpEF, commonly referred to as diastolic HF, have symptoms and/or signs of HF and a left ventricular ejection fraction (LVEF) >40% to 50%. The prevalence of HFpEF exceeds 3 million in the United States alone, and its prevalence seems to be increasing because of factors such as increased diagnostic awareness and greater longevity. The prognosis for patients hospitalized with HFpEF is poor, worsens with increasing age, and has not improved over time. Epidemiology studies have shown similar mortality rates in HF with reduced ejection fraction (HFrEF) and HFpEF; 1-year mortality is 26% in HFrEF compared with 22% in HFpEF. Transcatheter Interatrial Shunt Device for the Treatment of Heart Failure: Rationale and Design of the Randomized Trial to REDUCE Elevated Left Atrial Pressure in Heart Failure (REDUCE LAP-HF I).
UNLABELLED Heart failure with preserved ejection fraction (HFpEF), a major public health problem with high morbidity and mortality rates, remains difficult to manage because of a lack of effective treatment options. Although HFpEF is a heterogeneous clinical syndrome, elevated left atrial pressure-either at rest or with exertion-is a common factor among all forms of HFpEF and one of the primary...

OBJECTIVE To assess the use of detachable coils as an alternative method to occlude interatrial communications after Fontan operations. DESIGN Descriptive clinical study of selected patients after Fontan operation with interatrial communications inappropriate for transcatheter umbrella occlusion. SETTING Tertiary paediatric cardiac referral centre. PATIENTS Seven patients after Fontan ope...
متن کاملCorrection of right to left shunt by transcatheter treatment of pulmonary stenosis and atrial septal defect: a case report.
Co-occurrence of atrial septal defect and pulmonary valve stenosis is a rare entity for adult population. It is possible to correct both the pulmonary stenosis and the defect in the atrial septum by transcatheter methods. We present a case of right to left shant due to severe pulmonary stenosis and atrial defect which was successfully corrected by transcatheter pulmonary valvuloplasty and closu...
متن کاملEffects of an interatrial shunt on rest and exercise hemodynamics: results of a computer simulation in heart failure.
BACKGROUND A treatment based on an interatrial shunt device has been proposed for counteracting elevated pulmonary capillary wedge pressure (PCWP) in patients with heart failure and mildly reduced or preserved ejection fraction (HFpEF). We tested the theoretical hemodynamic effects of this approach with the use of a previously validated cardiovascular simulation. METHODS AND RESULTS Rest and ...
متن کاملA transcatheter intracardiac shunt device for heart failure with preserved ejection fraction (REDUCE LAP-HF): a multicentre, open-label, single-arm, phase 1 trial.
BACKGROUND Heart failure with preserved ejection fraction (HFPEF) is a common, globally recognised, form of heart failure for which no treatment has yet been shown to improve symptoms or prognosis. The pathophysiology of HFPEF is complex but characterised by increased left atrial pressure, especially during exertion, which might be a key therapeutic target. The rationale for the present study w...
متن کاملIntegrated Percutaneous Atrial Septal Defect Occlusion and Pulmonary Balloon Valvuloplasty
Introduction: Atrial Septal Defect (ASD) is one of the most common congenital heart diseases during childhood. Today, ASD closure is done by occlutech device via cardiac catheterization. ASD repair with transcatheter technique has shown high closure rate. However, coexistence of severe pulmonary stenosis and large defect in atrial septum is rare. When these two problems coexist, the result is i...
